Sometimes I will use the whole pillow depending upon my inclinations. The only time I elevate my head is when I am reading in bed. <br /><br />I do know that many actors don't feel that dialogue or background is as important as control over facial movements, head angles, body language, and composure. Overall, most focus is put onto the upper regions of the body, most notably the face/head and shoulders. Of course, I am talking of screen actors; stage actors have a whole different approach to acting due to the nature of the medium (but it includes some aspects of the screen-actor's toolkit). <br /><br />The film industry is pretty face-centric, all the way from agencies, to casting, to acting technique, to framing of shots, to marketing, etc. There are certainly exceptions, but like any art there is a tendency toward certain formulas. Overall, I think that your simple conclusion is correct, they are attempting to get a better shot of the actor's face.
